Purpose and Use of Inquiry Form
Use of this form is for anyone who is associated with a federally insured financial institution (e.g., a bank director, officer, employee, trainee, agent, etc.) who has a question or problem regarding the proper interpretation and application of any federal law or regulation that is administered by any of the five Federal bank regulatory agencies (i.e., FDIC, FED, OCC, OTS, and NCUA) that examines and supervises the institution for compliance with the federal law or regulation in question.

(NOTE: The Firm cannot respond to complaints against banks or private disputes involving banking transactions. See Special Disclaimer and Practice Limitation)

Inquiries may also pertain to any aspect of the conduct of any type of regulatory examination conducted by the Federal banking agencies, as well as any question or concern regarding the regulatory enforcement policies and procedures that have been promulgated by any of the Federal banking agencies to augment their examination and supervisory responsibilities.

Bank counsel and any licensed attorney dealing with a Federal bank law regulatory issue is specifically invited to make an inquiry regarding that question. [The Firm does not bill for any time answering an initial inquiry.]

Nature of Inquiry
Please describe the nature of the problem or question, including a brief summary of the circumstances under which it arose. If possible, please also identify any law or regulation that you believe may be (or have been told is) applicable to the situation. Further, it is often helpful to know when the problem or question first arose, and the nature of any actions taken to resolve the matter since that time.
